It is the intention of all of us at South Crossing Veterinary Center to provide your pet with a healthy,

secure, and comfortable boarding experience. We strive to satisfy your needs and to treat your pet as

we would our own.

Please be aware of the following policies and conditions:

     1. Drop off times are Monday, Wednesday, Friday 8:00 am till 6:00 PM, Tuesday, Thursday from 

         8:00 am till 8:00 PM Saturday 8:00 am until 1:00 PM. No Sunday service is available.

     2. Pick up time is by 10:00 am. If you pick up after 10:00 am you will be charged an additional ½

         day fee.

     3. We make every effort to accommodate your pet's comfort. Please feel free to bring toys, blankets

         etc.  However we will not take responsibility for loss or damage to these articles and we will not

         be held liable for injuries inflicted by these objects. This includes carriers, leashes and collars etc.

     4. We feed Hills "Science Diet" brand foods. If your dog is on a different food, we recommend you

          bring the food your pet is used to.  Being in a boarding situation is often stressful enough without

          adding diet change too.

     5. Some animals do not do well in a boarding situation. This may be because of stress, fear,

         aggression, or medical conditions.  We reserve the option to decline boarding your pet at our

         discretion. We are not responsible for any problems that arise due to preexisting conditions.

     6. To ensure the protection of all of our guests the following vaccinations are required to be up to

         date:  Dogs: Distemper, Bordatella, and Rabies.  Cats:  Distemper, Leukemia, and Rabies.  

     7. For pets receiving daily medications while boarding, please note that a medicating fee will apply.



One of the advantages of boarding your pet at a Veterinary Hospital is the immediate availability of

medical care as well as staff that is trained to recognize and deal with medical problems. If your pet

experiences a medical problem while boarding, we will attempt to reach you to consult and treat your pet.

If you cannot be reached to make decisions we will treat your animal within the following

monetary guidelines.

$100.00  $200.00  $500.00   Whatever is necessary to maintain the health and life of my pet(s)



  I do not authorize any medical care for my pet.  By electing this option I assume and accept all

responsibility up to and including the death of my pet and agree to hold all other parties blameless by my

decision.



I, the undersigned state that I am the owner or authorized agent of the owner of the below listed

animal(s) and I acknowledge and accept the above policies and elections. I further agree by my signing

to not hold South Crossing Veterinary Center liable for unforeseen accidents or injury to my pet beyond

reasonable control.
